Fixed Surface Pressure and Temperature

When the ``atm_option`` control has the value
``'fixed_Psurf_and_Tsurf'``, MESA sets :math:`P_{\rm surf}` and
:math:`T_{\rm surf}` to the values specified by the
``atm_fixed_Psurf`` and ``atm_fixed_Tsurf`` controls, respectively.
